  • Ed will have draft of nom process out to group for review by end of the week – hopes to present to board at April meeting.
  • Sandy Stuart here from HTA. Vocab group doing stringent cleanup of V2 tables, then align them with SNOMED. Ed: Are there terms exclusive to US Realm? US extension to SNOMED to describe things unique to the US – no equivalent in LOINC. Does the US ignore the rest of the world when creating value sets? Sandy – that would be a question for NLM. Meaningful Use in SNOMED – every time something goes through the SNOMED process, it is evaluated for US realm vs. Universal. Some projects using HL7 products that are trying to bring together the international community. Original discussion centered around race and ethnicity collection in US and that the practice is unique. Need to determine what is important clinically. Increasing number of DMA data contained which gives us more detailed definitions. Another issue: is it gender, or sex? Ed suggests to Sandy that if issues come up to work through those with us.
  • Paul mentions that there are a number of US-specific vocabularies used in FHIR – race and ethnicity are the key ones. They’re wanting to add “unknown” and “asked but not known.” Those to values are not represented in the standard value set for race in V3. What do we do about that? One approach would be to have a value set that absorbs the complete V3 race code set plus two unknown codes of standard origin. V2 and FHIR would use value sets. Ed: How do we move it forward? Will take a mandate from TSC, or topic for the new governance board. This is something we need to propose a pathway for – Vocabulary should be the next step in managing this issue. Two pathways: we can either push the issue to vocabulary, or push the issue back to FHIR to create a value set with two unknown values. Ed suggests Vocabulary is an appropriate structure and pathway for this and future issues.
    • MOTION to move this issue to Vocabulary to determine the appropriate approach to accommodate unknown in a value set with the version 3 race codes and create FHIR tracker item for this that is assigned to Vocabulary by Paul; second by Austin.
