- Several events can trigger an assessment, including admission, surgery, a lapse of time, etc.

The clinician makes observations necessary to assessing the patient's risk of developing a pressure ulcer. These observations may be made directly on the patient, they may be taken from devices, or they may be taken from the patient's record.
