- Several events can trigger an assessment, including admission, surgery, a lapse of time, etc.
- Main scenario
The clinician makes observations necessary to assessing the patient's risk of developing a pressure ulcer. These observations may be made directly on the patient, they may be taken from devices, or they may be taken from the patient's record. The attestor (who may be the clinician) signs or otherwise attests that the assessment is complete and correct.
