Project Summary

The Care Plan structure is used to define the management action plans for the various concerns identified for the target of care. It is the structure in which the care planning for all individual professions or for groups of professionals can be organized, planned and checked for completion. Communicating explicitly documented and planned actions and goals greatly aids the team in understanding and coordinating the actions that need to be performed for the person. Care plans also permit the monitoring and flagging of unperformed activities and unmet goals for later follow up.

Discussions at Phoenix WGM - January 2010

The (informal) discussion during Wed Q3 & Q4 discussed how to move the Care Planning in the context of HL7 work forward. It reviewed (at a high level) the IHE work, existing HL7 (within CDA and Care Provision)work and came up with a proposed set of action items:

  • Create Project Scope Statement
  • Define the topic to cover 'Care Plan', 'Plan of Care', ' Care Pathway' etc
    • References:

      • CEN Contsys
  • Validate existing care delivery approach in multiple domains to ensure it is generically applicable
  • Review and update:
    • Storyboard(s)
    • RMIM (ensuring Clinical Statement conformance)
    • Use cases
  • Review HITSP C162 and C83
  • Ensure international considerations are supported
  • Evaluate IHE PPOC workflow/dataflow model

Discussion issues are:

  • It is about the development of a framework of the care plan (problem solving)?
  • How to document and model the framework?
  • What do we want to send? the structure of the Care Plan, the outcome of the Care Plan etc? The uses cases that must be developed will give directions.Which messages?
  • Will it be a CDA document or is it part of Care Provision or both?
  • Is it a project of Patient Care or of Structured Documents?
  • Look at Diabetes work

Continuation of PC work on Care Plan

Patient Care workgroup will continue this Topic as follows: 1. Create the Project Scope statement to reflect the current approach (SAEAF compliant) 2. Create the DAM for care plan with the 4 parts described above (definition, use case, dynamic model, structural model). Identify the relationship with DCM. 3. Carry out the R-MIM update based on the DAM and reballot 4. Work jointly with SD on a CDA for care plan. This can be related to IHE PPOC, which has been agreed between IHE and HL7 PC to be done jointly.


Joint with SD 1 and 2. 3 is PC, 4 is SD and PC.

Motion: accept the above continuation for PC work on Care Plan as a project plan. moved Rosemary, 2nd Charlie. Against: 0, abstaining: 0, in favor 8.

Second Wednesday in March PC will have a conference call about the project scope statement. Before that update the project scope statement by William and reviewed by Rosemary and Laura. WG will set up the conference call.

This proposal was discussed on Q2 Thursday in a joint meeting with Templates and Structured Docs workgroup. PC will start creating the project scope statement. It might be better to have more, one for each item. Structured Docs is happy to join in this work, in particular the scope statement, domain analysis model and CDA document for discharge.
