- Patient id and document id are the same; ACS does not de-duplicate. Implementer can use patient ID, record ID, or any other method to generate an identifier, as long as it's unique to the facility.
- Time interval inclusiveness: comment was from V3 XML ITS CDA class (that to include a day in the "high" attribute, specify 11:59 pm; otherwise it's excluded).
