The pharm: extension for ingredient also contains quantity. The underlying nominator and denominator are also in pharm: namespace. This is inconsistent with the IHE implementation guide: http://www.ihe.net/uploadedFiles/Documents/Pharmacy/IHE_Pharmacy_Suppl_PRE.pdf, section 6.3.4.1.3.
It also seems to make sense to keep nominator and denominator in the 'normal' HL7 namespace and make (re)use of the appropriate HL7 datatype.
