We reviewed the sample survey forms from Survey Monkey and Google Forms today. We agreed to use Google Forms because we can have multiple form editors and users/editors of the sheets that it produces when we are finished.

Holly Miller, Russ Leftwich, and Bob Dieterle agreed to develop a short (5-minute) survey which we will review next week.
