Update XCN, XPN, XAD, XTN and CWE fields currently having sequence-based meaning.

Justification

This is a resubmission of a portion of the original proposal #514 and is a joint proposal from Quest Diagnostics and Kaiser Permanente seeking to take full advantage of the structure of the XTN data type. If XTN.2 and XTN.3 are required, there is no longer a need to have separate fields for the various telecommunication types: home phone, business phone, email etc. This also eliminates the need to have sequence based semantics to understand the meaning of multiple iterations of information. This is important because currently many fields have been assigned sequential meaning such as "primary number" that cannot be understood outside of context. XTN.4 should be made conditional in the component table to match what is stated in the component Note.
