Add components to the CE/CR/CWE/CNE data types to allow identification of the value set and value set version a code is drawn from

JUSTIFICATION DETAIL:

The CDC has identified an need to communicate an id for the value set and value set version that a code has been drawn from. Currently, the CWE and CNE data types allow identification of the coding system and code system version a code is drawn from. This proposal is to add components to the CWE and CNE data types to allow identification of a value set (using an OID) and the version of the value set. Both these new components should be made consistent with how they are to be handled in V3 data types.

OPEN ISSUES

There is a proposal to add a third triplet to the CWE/CNE data types. The new triplet (certainly a misnomer by now), should contain the value set id and value set version components also.
