HL7 Harmonization Proposal - V3 AdministrativeGender
Today's meeting was primary centered on the introduction of a harmonization proposal for the November cycle to reconcile the HL7 Version 3 AdministrativeGender concept domain and value set. The intent is to deprecate the Undifferentiated (UN) concept, as its definition implies clinical aspects of an individual ("The gender of a person could not be uniquely defined as male or female, such as hermaphrodite") as opposed to the intent for this concept domain to be used for administrative purposes only ("The gender of a person used for administrative purposes (as opposed to clinical gender") e.g., placement of a patient in an inpatient room.
Serafina presented an overview of the HL7 Harmonization process and the draft harmonization proposal that was submitted in order to meet the November Harmonization cycle deadline.
The HL7 V3 AdministrativeGender concept domain/value is used extensively throughout HL7 V3 artifacts (CDA and V3 messages) and may therefore present a challenge to work through this proposal during a single harmonization cycle. The "confusion" around this data element/concept is highlighted in the Meaningful Use Stage 2 final rule (see pages 54178, 54208-09) which supports the need for this proposal.
The work group discussed the fact that even if the proposal was not complete by the process deadline (October 28), we will learn a lot in terms of the history of this value set and its use by submitting the proposal, so the effort is worthwhile even if we withdraw the proposal this cycle in order to develop a proposal that will successfully meet all needs.
Prior to submitting the final version of this proposal, reference to version 2 user table 0001 - Administrative Sex will be removed since Version 2 tables are not considered in the HL7 Harmonization process and therefore is unrelated to this proposal.

Behavioral Health Summary project - next steps
Richard interested in cross social (HHS) systems data standards. SAMHSA grant programs cross a number of agency silos and there within SAMHSA, there is an increasing awareness that it makes sense to attempt to bridge these areas in an effort to improve care delivery and health outcomes.

- Information from this agency helps to support early intervention and prevention
- FERPA - rules related to protecting privacy that could be of interest to DS4P. Richard is hoping that the FERPA functionality will help to facilitate exchange between the silos of education and behavioral health care delivery
