FMM levels to:
- pages that are implementable (REST, JSON, Search, etc.)
- Profiles need to have FMM levels
- Implementation guides
- Code Systems & ValueSets can but don't need to
- In general - FMM lives at the top-level resource and cascades down, but can be asserted on lower levels if:
- lower level is re-usable across multiple parents
- lower level is independently implementable and has different implementation experience than its siblings
