  • Utah http://health.utah.gov/irb/ is expanding consent information requirements
  • Consent to treat and what the essence is that is to be captured and presented.
  • Child Assent for treatement is important to differentiate
  • In general, for treatement and other medicatl consent, all the information leading up to the ceremony isn't part but the result is what is sent with the source document/etc. attached.
  • Research Office for Human Research Protection https://www.hhs.gov/ohrp/ would be helpful and get their concepts.
