Example:

Background
Present any history necessary to understand the risk.
Example:
As the dinosaurs became more successful as a group, they grew larger and larger as competition between prey and predator increased. As successive eons have passed, the size increase has caused individual animals to become highly dependant on large amounts of food in the supply.
Indicators
Why the risk is relevant, examples of the risk being realized.
Example:
On numerous ocassions, small localized disasters have caused all dinosaurs in the area to be destroyed.
Impacts
List possible outcomes, worst case scenarios are acceptable.
Example:
A sufficiently large enough natural disaster could cause the collapse of the entire monophylum.
Mitigation Plan
List the various actions that will/should be taken to reduce the chance that this will occur or to reduce the impact when the risk does occur.
Example:
Store large amounts of biological material in amber so that enterprising scientists in the future can recreate the dinosaurs.

Any notes or comments from the Workgroup or things that occurred in the execution of the mitigation plan."
