QA excercise should be undertaken to review all references to Practitioner, and see where the gaps still remain.
The desire is to either have just Practitioner where the Org isn't important, or a set of properties (Prac,Role,Org) that cover this use case (such as on CareTeam) or in other cases, Practitioner|PractitionerRole as optional reference types.
