Review Stephen's model on how all the artifacts of Care Plan relate. Feedback provided - Stephen will take the feedback and adjust the diagram for future review

Discuss advance directive issue brought by Lisa Nelson related to change in decision for care to be provided when case progresses. After lots of rich discussion, Lisa will take the feedback to the team and work on an alternative plan to bring for review at a future meeting. Will start by looking at the instruction template -

update from Evelyn Gallego - "I am still working with the NIH NIDDK team on the Chronic Kidney Disease Care Plan. It is unclear whether they will move to create an IG around it."

Mike Padula - Review of the current mapping of the Child Health Data elements to the CCDA templates to get an understanding of the issues he is running into - Suggested he does send out the spreadsheet for the team to be able to provide more detailed feedback. Plan to look at this at next call with Mike.
