      1. in light of Stan's pulse example: should this be more complex, or should we assemble atoms into molecules?
         1. assumption: composition. 5 atomic skin inspection component obs; one aggregation.
         2. everyone on call agrees

   2. in light of Claude's contextual axes,
      1. does skin inspection have an order state?
      2. does it have an assertion kind?
         1. these matter one level higher than our skin inspection level
         2. assertion would be different semantics (pt has dry skin, not 'dry')
         3. this style works especially for Q/A concepts

   1. may need atrophic (thin) value for condition
   2. are condition & moisture duplicative?
   3. what would this be in FHIR? observation probably

   1. Stan: could have used LOINC codes for model binding
   2. IM style: no code at model root. unique id for model. with relationshps
      1. then code = concept (L or SCT. either way)
