Appendix C can stay as is - point to where additional extensions are listed on SDWG. Appendix D - keep only what is relevant (confirm with Brett and Keith). Reference the source material. Appendix E - Include why it is relevant and how it is different from a Personal Advance Care Plan. Also reference the source material.
