Vital Records Standards Initiative

  • The Centers for Disease Control and Prevention/National Center for Health Statistics (CDC/NCHS) has provided support for the development of vital records standards to enable interoperable electronic data exchanges among electronic health record systems, United States (U.S.) vital records systems and potentially other public information systems for birth, death and fetal death events. The CDC/NCHS, the National Association for Public Health Statistics and Information Systems (NAPHSIS) and several state representatives have collaborated to develop data models, functional profiles, messaging and document standards for vital records with primary support from the HL7 Public Health and Emergency Response Work Group.

Birth and Fetal Death Reporting
HL7 Version 2.5.1 Implementation Guide: Birth & Fetal Death Reporting, R1 (US Realm - Draft Standard for Trial Use) This implementation guide is an initial effort to provide guidance and messaging infrastructure for transmitting medical/health information on live births and fetal deaths from a birthing facility setting to a jurisdictional vital records electronic registration system. The use case describes the transmission of the data using trigger events and abstract messages to record the creation, revision, or retraction of live birth or fetal death reports.
HL7 Version 3 CDA R2 Implementation Guide: Birth & Fetal Death Reporting, R1 (US Realm - Draft Standard for Trial Use) This implementation guide provides a standardized format using HL7 Clinical Document Architecture (CDA) R2 to transmit medical/health information on live births and fetal deaths from a birthing facility setting to a jurisdictional vital records electronic registration system. Medical and health information collected from Electronic Health Record (EHR) and data for the birth certificate and fetal death report once gathered, can be provided to public health agencies to track maternal and infant health to target interventions for at risk populations.

HL7 Version 2.5.1 Implementation Guide: Vital Records Death Reporting, R1 (US Realm – Draft Standards for Trial Use) This implementation guide is an initial effort to provide guidance and messaging infrastructure for transmitting death related information from a clinical setting to the vital records electronic registration system. The use case describes the transmission of the data collected using ADT messages to address a specific public health purpose.
HL7 Version 3 CDA R2 Implementation Guide: Vital Records Death Reporting, R1 (US Realm - Draft Standard for Trial Use) This implementation guide provides a standardized format using HL7 Clinical Document Architecture (CDA) R2 to transmit death related information from a clinical setting to the vital records electronic registration system.
Functional Profile
HL7 EHR-S FM Vital Records Functional Profile, Release 1 The Vital Records Functional Profile (VRFP) conforms to the HL7 EHR-S FM. The HL7 EHRS FM defines a standardized set of the functions that may be present in EHR systems. The VRFP will facilitate the point-of-contact or point-of-care capture of selected vital records data via EHR systems. The VRFP project is U.S. focused and will initially specify the functional requirements needed to support messaging of selected U.S. vital records data among providers, states, local registrars and Federal agencies.
Data Model
HL7 Version 3 Domain Analysis Model: Vital Records, Release 1 The Vital Records Domain Analysis Model (VR DAM) includes activity models of typical birth, death and fetal death processing in the U.S. utilizing flow diagrams to identify the work flow processes, stakeholders and data exchanges that support data collection, record registration, and certified copy issuance. The VR DAM also includes birth, death and fetal death class models that depict the vital records data captured and the data relationships. The Vital Records Storyboards provide narrative descriptions to illustrate typical scenarios in which vital records information is collected and recorded on the U.S. Standard Certificate of Live Birth, U.S. Standard Certificate of Death and a U.S. Standard Report of Fetal Death. These storyboards are not intended to represent all vital records processes or circumstances.
