Health Story: Integrating Narrative Notes and the EHR (formerly CDA4CDT)
Health Story is an alliance of healthcare vendors, providers and associations that pooled resources over the previous three years in a rapid-development initiative to produce data standards for the flow of information between common types of healthcare documents and electronic health records.

Project Status
In 2011, all Health Story standards and the Continuity of Care Document (CCD) were consolidated into single Consolidated CDA Templates DSTU. This DSTU is a single place to find all relevant templates for the document types: CCD, Consultation Note, Discharge Summary, History and Physical, Imaging Integration, Operative Note, Procedure Note, Progress Note, Unstructured Document.
The Consolidated CDA templates DSTU demonstrates the templated CDA architecture. Future projects will enhance the Consolidated CDA Templates DSTU.
