Issue

When one uses the Encounter R-MIMs as present in PA's domains, and wishes to identify the ServiceDeliveryLocation the RoleCode coding system doesn't contain codes for the most common service delivery location types: unit, room, bed. There are use-cases where one has to send all three, associated by a part/partOf RoleLink (see below).

Vocabulary Recommendation(s)

The codes are from the RoleCode coding system; in this case we want to add new concept codes to the DedicatedServiceDeliveryLocationRoleType value set:

1. HB : "Healthcare Bed": the usage of a bed as a practice setting where diagnostic procedures or therapeutic interventions are performed. This includes concepts such as gurneys in the hallway to acute critical care beds, and bassinets in nurseries.
2. HR: "Healthcare Room": the usage of a room as a practice setting where diagnostic procedures or therapeutic interventions are performed
   • ER is the only extisting concept code which uses the description 'room'
3. HU : "Healthcare Unit": the usage of a unit as a practice setting where diagnostic procedures or therapeutic interventions are performed. This includes the concept of ward.
   • HU already exists (InternalID 10596) for 'hospital unit'

NOTE - the following coded concept already exists in the value set

• DX "Diagnostics or therapeutics unit" - A practice setting where diagnostic procedures or therapeutic interventions are performed

Rationale

• When the use-case was floated on the PA list it was pointed out that Bed and Room could be physical locations (the playing Entity of a ServiceDeliveryLocation).
  • If we take the 'room' identified as "operating room #2" then this is a functional name, which happens to be in (physical) room 2507 today, but could be (after building a new wing of the hospital) room 9803 tomorrow.
  • Likewise, 'ICU bed #5' could be one physical bed in a particular room today, and another physical bed (and location) tomorrow.
  • The description of the new codes is done in a manner which clarifies that we're talking about the 'functional role', and not of the underlying physical entity.
