- Assessment Problem from "A"ssessment and "P"lan (of SOAP) -- includes diagnosis
- Problem on Problem List -- if homeless or financial barriers or lack of transportation are being managed, then it could be a problem; clinician expresses concern about social history; can be patient-stated concerns (doesn't need to be on a clinician's problem list) such that asserter will differentiate between them -- could include risks / fears as well.
