QDM Based HQMF R1.3

• Previous versions of the QDM-based HQMF (before R1.2) included a table of QDM Categories, Datatypes, and Attribute TemplateIDs. It will be helpful to have a table in this version to show the transition between QDM 4.1.2 and 4.2. The Github project has the new ones added but a table indicating versions would be helpful to implementers. In particular the same template ID in the Github registry is used for two different QDM attributes: Onset Age and Result. Clarification will be helpful. (Submitted by Floyd Eisenberg on behalf of C.
