Summary

Clinicians face numerous knowledge needs during the course of patient care and the majority of these needs are not met, compromising the quality of care. Likewise, in order to be partners in the decision making process, patients need to be well-informed about their health and health care options. Online health knowledge resources that are capable of solving clinicians’ and patients’ knowledge needs are now widely available, but a series of barriers hinder a more effective and frequent use of these resources to support decision-making. Context-aware knowledge retrieval into Clinical Information Systems (CIS), such as Electronic Health Record (EHR) and Personal Health Record (PHR) systems, is an increasingly promising approach for delivering relevant clinical knowledge to the point of care as well as patient-tailored educational material to support patient-centered care. These kinds of knowledge retrieval tools have been known as "Infobuttons."

To support the integration of knowledge resources into CISs, the Clinical Decision Support Work Group (CDS WG) has been developing a set of standard specifications for context-aware knowledge retrieval. The first of these specifications, entitled Context-Aware Knowledge Retrieval, Knowledge Request Standard, was approved in September 2010 as a normative ANSI/ISO HL7 standard. This specification provides a standard mechanism for Clinical Information Systems to submit knowledge requests to knowledge resources. In addition, a URL-based implementation guide has been developed to specify knowledge request implementations using the HTTP protocol.

Description

An 'infobutton' is a point-of-care information retrieval application that automatically generates and sends queries to on-line health information resources ('e-resources') using patient data extracted from the electronic medical record and background information ('context') that is captured from the interaction between a clinical user and a clinical information system (e.g., user role, patient age and gender, task being performed by the user). Currently, e-resources typically provide HTTP-based Application Program Interfaces (API) that can be used by infobuttons. However, these APIs are based on proprietary syntax and vocabularies, requiring the development of custom software for each e-resource that an infobutton needs to link to. The goal of this proposal is to facilitate the implementation of infobuttons by supporting the integration between Clinical Information Systems and e-resources.

The Knowledge resource response to an infobutton knowledge request message payload for this interaction is outside the scope of this specification.

Summary

The approach outlined in this implementation guide allows Infobutton event notification and information resource event notification message payloads to be converted from an XML format to a URL-based format using a simple automated process. Specifically, the implementation guide's rules allow an XML-based Infobutton message model message payload to be converted to a set of parameters that can be included in an HTTP request using GET or POST methods.

The main goal of this implementation approach is to support compatibility with most EHR and knowledge resource implementations, maximizing adoption. This document assumes knowledge of the Context-aware Knowledge Retrieval (Infobutton) standard normative specification.

The intent of this recommendation is to support backwards compatibility with the majority of the infobutton and information resources that exclusively support URLs as the communication protocol. The ultimate goal is to enable a stepwise transition from URL-based implementations to an HL7 Implementation Technology Specification (ITS). Nevertheless, it is not the intent of this document to create a URL-based HL7 ITS.

Summary

This specification aims to provide guidance regarding the implementation of context-aware knowledge retrieval applications in a Service-Oriented Architecture based on the HL7/OMG Decision Support Services (DSS) Standard. The specification includes: 1) Functional requirements for context-aware knowledge retrieval in a SOA; 2) Technical specification for the operations and semantic profiles (service payloads) to be supported by Context-aware Knowledge Retrieval DSS implementations.


Description

The SOA Implementation Guide for context-aware information retrieval ('Infobutton') leverages the HL7 Decision Support Services (DSS) standard. More specifically, the scenarios and interactions defined in the proposed specification will be implemented as a DSS 'knowledge module.' This project focuses on the specification of a DSS module to support a SOA-based integration of knowledge resources into electronic health records. The proposed implementation guide will not specify the payloads of the Infobutton DSS knowledge module. The knowledge module payloads have been / will be defined elsewhere in HL7 normative specifications, including the Context-aware Information Retrieval (Infobutton) DSTU. These work products will be developed independently of, and in parallel with this SOA implementation guide. 1) The SOA IG will be an instance (a.k.a., knowledge module) of the broader DSS Standard. 2) Unlike the present Infobutton DSTU (Project 265), the SOA IG will support back and forth communication between EHRs, Infobutton Managers, and knowledge resources. Resources will be able to respond in a standardized fashion, which is not supported by the present Infobutton DSTU. 3) The Infobutton DSTU will serve as the payload for knowledge request operations in SOA implementations. 4) SOA-enabled knowledge resources will respond with a knowledge retrieval metadata payload, which is being developed in parallel to the SOA IG
