The FHIR Substance example in section 3 is incorrect: the SNOMED CT Code System should be http://snomed.info/sct
Note that the full URI of the code is correct: http://snomed.info/id/406466009, but that you cannot just concatenate the (SNOMED CT) system, version, and code values to produce a valid (SNOMED CT) URI for a (version of) a concept.
