Change Requests for PHER ELR IG Release 2

Release 1 of the IG was named in the US Meaningful Use final rule, and implementers are now building to it.

Therefore, an ELR Release 1 Errata has been produced and will be limited to fixing errors, adding clarifying text, and updating examples.

Additional Errata and Clarifications were produced and the documentation source material are listed below:

No substantive changes will be made.

Requested changes not added to the Errata will for the basis of Release 2.
