CPM Storyboards Immunization

Jeremy arrived at His HMO with a card listing his immunization history. He had received 3 DTaP, 3 polio and 3 HIB/Hep B. The dates given were recorded on the card. The card came from the local Public Health agency. The nurse recorded this information in the EMR. She also gave him 3 immunizations:

PCV7 (lot # 1234rj) this is pneumococcal vaccine, dose size was 0.5 ml, site given R vastus lateralis, prescribed by Dr Who MMR II (lot # 432emd) this is vaccine against measles, mumps and rubella, dose size was 0.5 ml, site given L vastus lateralis, prescribed by Dr Who Pentacel (lot # 78u, lot # y432) this is vaccine against DTAP, polio, HIB, dose size was 0.5 ml, site given R vastus lateralis, prescribed by Dr Who

He is currently eligible for vaccine from the Vaccines for Children program (VFC)
