   1. AAAM - no reponse. We'll try to find LOINC rep at HL7 next week. Plan B may still be necessary (ACS code).
   2. Example does not yet show all data; plan will complete this (& guide updates) for next meeting (9/22)
   3. Using GNIS: States are not in GNIS; continue to use ANSI FIPS for states
