Health Concern Topic

Patient Care WG October 24, 2013

Discussion of the Patient Care projects – Health Concern, Care Plan/Care Coordination Services and Allergy and Intolerances.

Michael – drafted a use case involving diabetes. Lisa questioned the addition of the patient to the concern. Kevin – is part of any health concern. Do not distinguish who raises the issue – we support all actors.

The relationship between concerns is not tracked – but will need to include in the model. What about other concerns – financial, spiritual etc? E.g. non-medical. We need use cases so that all types of concerns are included.

Michael – will collect the use Types of Use Cases (user stories) needed:

  1. From various points of view (Care giver/ Patient)
  2. How individuals interact with system
  3. Tracking a concern over time
  4. Transitions of care

Can we use the storyboards from the care plan project? Can update to demonstrate how the health concerns interact with the care planning process. Note that the use cases serve as a basis for modeling.

CIC has other projects that we may use. (Note that a formal relationship was not on the project scope statement).

  1. Immunizations
  2. Cardiovascular Imaging
  3. Schizophrenia
  4. Major Depressive Disorders
  5. TB (Adult and prediatric)
  6. Pre-op Anesthesia
  • Lisa will contribute a use case for the patient-based concerns.
  • Kevin – acute care setting.
  • Stephen Chu – will also provide use cases.
  • Elaine – will provide a nutritionally based use case.
  • Laura and Elaine will take a look at the use cases related to the Care Plan.

Lisa – providers may choose a path based on health concern.

Next conference call in two weeks. Please send to Michael for collation at latest November 4th.

Does a concern always need a condition, for example risky behavior? How do you capture at risk for, potential for and they are tracked over time. Also needs to be more broad than physicians because of interdisciplinary care.

It may be that the standard condition model does not work. Need an observation to create the concern.

Next Meeting – Thursday, November 7th. Will review the collected use cases.
