Issue
Table 6-2 description for PID-3 and PID-5 as mandatory does not line up with CLIA definition. CLIA defines these as (either or) not both.

- 21-Jun-2012 - Riki to check with Megan at CDC on what is required. We think PID-7 should be included and that 2 out of 3 are required. We will then note that Usage in PID already ensures this.
