Update of Continuity of Care Document for use in CDA r3
This is a joint project of Patient Care and Structured Documents to provide an update to the Continuity of Care implementation guide suited for use in CDA r3, based upon the current Clinical Statement.

- Name the project facilitators, (add contact info if desired)

- If balloting, list ballot name/cycle/link
