Agenda:
- Review Stephen's model on how all the artifacts of Care Plan relate. Feedback provided - Stephen will take the feedback and adjust the diagram for future review
- Discuss advance directive issue brought by Lisa Nelson related to change in decision for care to be provided when case progresses.
- update from Evelyn Gallego - "I am still working with the NIH NIDDK team on the Chronic Kidney Disease Care Plan. It is unclear whether they will move to create an IG around it."
