Here is the initial set of forms targeted for this project:

• Surgical Site Infection (SSI) Form

• Denominator for Procedure Form

• Primary Bloodstream Infection (BSI) Form

• Denominator for ICU, Other locations (not NICU or SCA) Form

The following forms will also need to be handled at a later date:

• Urinary Tract Infection (UTI) Form

• Pneumonia (PNEU) Form

• Denominators for Specialty Care Area (SCA)

• Denominators for Neonatal Intensive Care Unit (NICU)
