• Surgical Site Infection (SSI) Form

• Denominator for Procedure Form

• Primary Bloodstream Infection (BSI) Form

• Denominator for ICU, Other locations (not NICU or SCA) Form

Here is the initial strawman CDA instance for the BSI form (for discussion only)
