When I get some time occasionally, I look at what has been registered and sometimes send emails to registrants if things look fishy. But sometimes it is months between times of me doing this. The approval process is of two types: a) for a code system, when I change the status from "pending" to "complete", I send an email that says they are good to go. The registration of identifier schemes and code systems will have to be checked for duplication like any other OID registered and if duplication does exist (as the HL7 OID policy included above states) until the duplication is dealt with, the HL7 OID shall be the preferred OID used. The Hl7UK realm specific policy states that if the message is within the UK profile then the HL7UK OID will be the preference.

OIDs have no meaning other than to uniquely identify. Identification and meaning are often confused by people but they are very different. I can identify a thing without knowing its purpose/meaning AND I can appreciate the meaning of something without uniquely identifying it. I can understand perhaps how the confusion arises since OIDs are also used to identify coding systems. However, the coding system identified gains it meaning from the link between codes and meanings within the system and not from the OID itself. It would also be possible to construct a coding system in which OIDs or UUIDs were used as the"codes" (or concept identifiers). However, in this case the code system would need to link each of the permitted values to a meaning. Furthermore, the resulting expression would go in a CV or CD datatype not in an II and whats more it would only identify a concept not the class it was in. Therefore, in my opinion you should not allocate an OID to a UUID. The use-case should be examined and met by a more appropriate mechanism.

If you care about the "meaning" of an identifier, then it should be on a separate Act or Role and the Role.code or other attributes should be used to convey that meaning.

If I can't tell the person's SDS User Id from their GMP Code except through the scoping organization or the role.code, then the UK's use of SET<II> is incorrect. I should only use a SET<II> datatype when the entity can have multiple identifiers of the same type (role.code) assigned by the same organization. I'm going to guess they should be using the "OtherIDs" relationship. In the current model of the UK interfaces, we need to use the OID to tell us the "assigning authority" and "identifier type" that was part of the v2 EI data type.

I believe the UK has the concept of "business identifier" as well as "version identifier" and even "view identifier" which allow referencing to specific versions and specific subsets of information about the role. Because we haven't provided separate places for these to be conveyed (yet), they may be using SET<II> to meet that purpose. Also, be careful about the assumption that the identifier is assigned by the scoper. Identifiers are *used* by the scoper, but could well be assigned by another organization. For example, a hospital might use SSN as their patient id (not recommended, but it happens). In that case, the identifier is *assigned* by the U.S. government, but the scoper would still be the hospital. SET<II> is much more appropriate when used on Entity, where it basically says "Here's a bunch of identifiers by which this entity is known", but no information is provided about whether the identifiers are patient ids, drivers' licenses, credit card numbers, insurance ids or whatever. Their sole purpose is to allow matching. It is technically possible to do a lookup for a particular OID to determine the assigning authority and 'type' of an identifier. However, systems should never depend on the receiver having this information. If it's important to know that a particular id is a driver's license, then this should be conveyed as part of the message.

I believe the use case that Penny referred to was such that an OID might be allocated to UUIDs used for a particular purpose (it would be pointless to add an OID just for uniqueness). This is clearly a wrong use of the root OID because it seeks to assign a meaning to the identifier. These is another issue related to identifiers of instances in different views (business, snapshot and templated-snapshot) which needs to be handled by HL7 and which is properly a topic of an active debate. This does need to be resolved but doing it through addition of semantic significance to an OID is not the way to manage this. It is incorrect, verbose and breaches a key element of standardisation. However this frustration and confusion is something that can only be overcome by providing models where it is clear where that information does come from, and by having a fruitful oid symposium to produce further clarity as to why II works the way it does

If multiple identifiers are present then the recipient can look for a match on any one of these. That is if they have an existing record of with the complete root/extension pair they have a match. So SET<II> works in a heterogeneous loosely coupled environment to allow exchange of information identified differently in two systems - provided that one reference system has both identifiers. I am not sure this is really a major use case but have heard it advanced in the past and it certainly has the potential to work. However of course for UUIDs it is irrelevant to have the OID even if there are multiple UUIDs as a simple match on the UUID is enough. The primary problem is the overloading of the idea or identifier. A sensible reworking of this would be extraordinarily helpful. A Instance identifiers of information objects: 1. Business process identifier - where an information object content can evolve without changing the ID 2. Snapshot identifier - where the information object contents represent information at a given point in time and are immutable 3. Templated identifier - where the information object contents as stated are complete and immutable (completeness being defined by a scoping template??) B Real world identifiers: 4. Named identifiers having a specific "purpose" (e.g. to "identify a person" or to "identify a business transaction in a given context" - such as a "claim number" or"referral number") and a specific "issuing authority" (e.g. "UK NHS")

However, those OIDs are not listed in the HTML rendition that is part of the 2005 Edition. As these OIDs are what people are supposed to use for CD.codeSystem, shouldn't they also be listed in the spec (and not just in RoseTree)? I note that they are also not in the XML export from RoseTree (which is most likely why they are not in the HTML, which is generated from the XML export :-). Am I just missing something here or is there something wrong?

- Paul and Tom, The issue is a tooling issue, and the lingering confusion borne of the (bad) practice in the past of naming a domain, value set, and code system with the exact same string of characters, thereby confusing all and sundry. A Domain is a semantic type, and is a reference to a non-rigorously defined constraint statement (prose). In the tooling it is used in the binding of a specific collection of coded values to specific items in a static model. The only reason a Domain could ever be argued to have an OID would be for convenience only, since we have machinery that deals with OIDs for globally unique objects. It would be nice if all the tooling and publications surfaced the OIDs easily so that you didn't have to check many places for the information you need, but again, that is a tooling issue. You can't 'know' the code system from the Domain, since there is a 1..n relationship between a domain and a code system. You have to go through the Realm and Value Set machinery to identify the code system. For complex structures in a message instance where you have Realm- specializable objects inside templates with different conformance profiles that are part of CMETs...well...you actually have to do some pretty fancy run-time evaluation of parallel streams of stuff to figure out whether or not a particular vocabulary term is valid in a particular place in a particular message instance. It took me many hours of discussions with Lloyd and Woody before I finally grasped what was going on. These relationships are NOT static, and cannot be directly expressed in a static structure, such as an excel table. We have also had issues in the past that we are still trying to get the last remaining bugs out of where the denormalized copy of the OIDs in the RIM Repository from which the ballot is derived and against which the tooling (such as RoseTree) run became desynchronized with the OID registry on hl7.org, which is the Source Of Truth for code system and value Set OID assignments. Only a handful of errors remain. We recently decided that all Value Sets would have OIDs, again for convenience primarily since the machinery is already in place. Note that the identification of a Value Set is NOT contained in a message instance 'on the wire'. We are spending time with the definitions because it is a fairly complex piece of gear, and everyone who is not immersed in vocabulary full-time seems to be getting confused by all the published documents we have so far. Volunteers to improve the documentation are welcome (and solicited).

I knew the difference between vocab domain, value set and code system, but can certainly understand why the difference is sometimes hard to explain to 'outsiders' (i.e. people that implement but don't design HL7 models and messages). In the end of course, implementers only care about code systems, and the OID that belongs to them needs to be present in their instances. Fact is that many vocabulary domains in the HL7 ballot have an associated code system in the universal realm (i.e. an HL7 defined code system). Wouldn't it be possible to link directly to the OID associated with that code system from the vocabulary domain (at least in those cases where there is an HL7 defined OID). The difference between the concepts would need to be clearly explained, but it would be very practical for the people who implement the messages we design. Just because things are conceptually different, doesn't mean we can't link them (while making it clear that the actual OID is dependent on the realm binding to the vocabulary domain).

- [Response from K Boone email 200512061148]: Iris, The information you are looking for is in the Hierarchical description. It appears in the Domain column in line 223 of POCD_HD000040.xls. That's the column you would use to determine vocabulary domain for any element in the CDA. Furthermore, the CS column indicates whether this is a CNE or CWE domain. It took me a while to figure this out, and I still don't like HDs and HMDs, but I've learned to live with them. It does make it difficult to validate an instance, because you've got to deal with large lists of vocabulary to really do the validation. That's why I'm using schematron to validate implementations as well as schema, because I can use a few XSLT tricks to verify that a particular element (e.g., healthCareFacility/code) uses the appropriate coding system and values. It gets a lot harder when the particular vocabulary domain is too broad (e.g., SNOMED CT), because you may actually want to restrict the code value to a domain subset, and right now, that's pretty difficult, especially when the vocabulary domain subset is not easily described. For example, relatedEntity/code in CRS is limited as indicated below: The value of relatedEntity/code should be present and indicate the type of healthcare provider. If present, the values shall be drawn from SNOMED CT, using concepts that descend from the healthcare professional subtype hierarchy (SNOMED CT Concept ID: 223366009). The problem is that SNOMED CT is the codeSystem, but the domain is a subset of SNOMED CT, and there's no easy way to represent that in an HD for vocabularies that are not controlled by HL7.

The reason the vocabulary people believe (and I believe) lack of specification of a specific value set for a value is poor vocabulary practice is that the interpretation of a value is context sensitive, depending upon the specific value set it is chosen from. Not to specify the specific value set means that the specific semantic of the value cannot be reliably interpreted by the receiver. Vocabulary people feel, in theory, that the addition of an individual code to a value set is essentially creating a new code system.

- 25.05[ Response from L Mckenzie 20051229]: One of the challenges will be whether the selection criteria available to the user actually included all of the codes specified by the valueset for the message, template, or whatever. There's some debate whether conformant applications are required to allow selection from the entire valueset. I can certainly see this as being theoretically useful, but am not sure whether it's something many (if any) implementers will be willing to take on.

Rather, we need to nail down the "requirements" for good vocabulary practice from the vocabulary committee and communicate those in implementation guides so that there is a "direct map" from theory to implementation.
- 25.07[response from L. McKnight 20051230]: I'm not much of a vocabularist, and I don't know the HL7 spec on this, but it would seem to me a problem comes with versioning which needs to be specified.
- If the codeSystem version is buried (say as the last digit) in the OID, then you do know quite bit from an incomplete OID. However, it would be difficult to know which coding systems did this, so you would probably have to ignore it.
- If you don't make a different OID for each version, however, in some cases it might get you into trouble. The classic example is code reuse in ICD.
If I don't know the version, I'm likely to misinterpret information. If this information is in codeSystemVersion, and that field is not required, you have a problem.
- The problem exists in well designed vocabularies too. For example, say System A has version 1 and System B has version 2. Version 2 adds code XYZ. System B sends a message containing XYZ to System A. System A looks at the codeSystem (a quick and easy thing to check) and says "I understand", and accepts the message. But then gets to the code XYZ and says "Hey, that's not a code" I can't use this. The implication is that all codes must be checked for confirmation of "I understand" at runtime which is a much harder job.

- The central dispute is about what I can know when you send me an unknown OID. Suppose you send me OID 123.456.789 and that is unknown to me...but I do know OID 123.456. What inferences: 1) can I make ; 2) am I allowed to make; 3) is it safe to make about the unknown OID.
- Some folks, if I understand them correctly, answer: 1) none; 2) none, 3) none.
- On the other hand, I answer: 1) I at least known the assigning authority of the unknown OID (because it is the OID that I do know)...and depending on what I know about that assigning authority I might be able to make other inferences (this is very similar to knowing that IP addresses in this building at KP/Pasadena begin with 10.209.144.x, so if you tell me to connect to 10.209.44.123 I know that that machine is in this building...I don't know exactly where in the building, but I know it is in this building) ; 2) I am "allowed" to make whatever inferences I want; 3) safety is a difficulty question and what is safe or not depends on characteristics of assigning authorities that I do know about.
- So, some folks want to say that OIDs are ONLY for disambiguating identifiers...and I say that an application is allowed (in fact,encouraged) to make certain (albeit, limited) inferences about unknown OIDs."
