PH Vital Records
The increasing prevalence of electronic reporting of vital records (birth, fetal death, and death) data has led to the need for standards of data exchange. These requirements are tightly tied to the nationally defined forms for reporting VR data. A comprehensive VR DAM has been developed and guides the development of standards in a variety of product families.
The Vital Records Domain Analysis Model (VR DAM) has been developed to ensure that it reflects the evolving functional and information requirements of comprehensive vital records birth, fetal death, and death reporting processes.

Birth and Fetal Death Reporting
Reporting of live birth and fetal death data is covered in both a V3 CDA implementation guide as well as a v2.6 implementation guide (which replaces a previous v2.5.1 IG). As of August 2017, the v2.6 IG is still in ballot reconciliation following the May 2017 ballot period.
This project also encompasses data contained on forms for the mother and birthing facility.
Reporting of death data is covered in both a v2.6 implementation guide as well as a V3 CDA implementation guide. As well, FHIR resources and profiles for death reporting are being developed to provide an alternative reporting format to the existing messaging and document options.
