FMM levels to:

• pages that are implementable (REST, JSON, Search, etc.)
• Profiles need to have FMM levels
• Implementation guides
• Code Systems & ValueSets can but don't need to
• In general - FMM lives at the top-level resource and cascades down, but can be asserted on lower levels if:
  • lower level is re-usable across multiple parents
  • lower level is independently implementable and has different implementation experience than its siblings
